Companionship

Supporting the Plan’s members with general companionship activities including…

Homemaker or Chores Support

Supporting the Plan’s members with help around the home through support that includes

  • Assessing the Health Plan’s Member’s residential environment
  • Preparing and serving meals
  • Changing and making the member’s bed and managing linens and laundry
  • Cleaning specific rooms of the home (e.g. kitchen, bedroom, bathroom, etc.)

Personal Care

All homemaker tasks, plus assistance with ADLs such as

  • Bathing, dressing, feeding, transferring, ambulation, and toileting

Assistance with IADLs such as

  • Transportation, errands, and grocery shopping
  • Medication adherence

Handyman Services

In-home evaluation and installation of any of the following safety devices by a qualified professional

  • Lever handle sink faucets
  • Handheld shower
  • Raised toilet seat 
  • Non-slip stair treads
  • Bathroom grab bar
  • Window air conditioner

 

Pest Control

In-home evaluation and treatment by a qualified professional
  • General insect treatment
  • Rodent infestation treatment
  • Pest control deep cleaning

Rides to Medical and Non-Medical Locations

Supporting the Plan’s members with rides to and from appointments through services such as

  • One-way rideshare ride
  • Medical sedan ride
  • Wheelchair van ride

Standard Grocery Bundle

  • Assortment of Meat, Dairy, Produce and Pantry items

Pantry Only Bundle

  • No Refrigeration, Assortment of Shelf Stable Pantry Items

Nutritional Partnerships

In addition to the curated grocery bundle options, The Helper Bees partners with the following grocery and meal vendors for a variety of options to provide members access to healthy food.

InstaCart Fresh Funds:

  • Fresh Funds is an Instacart program that allows plans to fund category-specific stipends, like fresh and frozen produce
  • Fresh Funds recipients can redeem via a digital code and shop for eligible categories on Instacart

Meals on Wheels:

  • One week of 10 meals delivered to the home with an option to add a “More than a Meal” check-in service

BeWell Nutritional Programs:

  • Consultation with Registered Dietician
  • Kick-start pantry kit
  • Monthly grocery delivery/meal delivery options with support and Registered Dietician consult

Virtual Exercise Programs

  • Access to a broad range of virtual, small-group exercise classes with themes such as yoga & chair yoga, mindfulness, balance, core strength, and stretching
  • Initial member assessment to establish baselines with follow-up assessments every two months.
  • Access to “community classes” where members can bring a friend
  • Strength bands and water bottles sent to the member’s home
  • Classes can be structured as an ongoing membership or as individual, a la carte sessions

  • Scale, thermometer, pill splitter, medication organizer, pulse oximeter, blood pressure cuff, first aid kit, face masks, hand sanitizer
  • Bottle of spray for common household insects, mouse traps 
  • Draft blocker, step stools

General Assistive Devices

  • Air purifier, ionizer, evaporated air cooler, dehumidifier, humidifier
  • Assorted bedding, sheets, comforter, blankets, scale, adult unisex bibs, pillows, mattress pad
  • Bed safety & assisting rails, overbed tables, disposable wipes or washcloths for adults, bed pans, bedside commode, urinals, toilet safety frames
  • Walking cane, cane accessories

Errands

Supporting the Plan’s members with a range of errands to the home that include

  • Basic, no purchase errand
  • Household items bundle delivery errands
  • Snow removal errand
  • Yard maintenance errand

In-Home Dental Services

  • Mobile equipment – digital x-ray units, custom dental chairs, wheeled dental units, and portable operatory units  – make it possible to thoroughly diagnose and perform dental treatment in different living environments
  • Services include exams, cleanings, fillings, x- rays, crowns, dentures, bridges, extractions, root canals, night guards, hygiene therapy and other dental procedures as clinically necessary

Virtual Physical Therapy

In-home treatment for all musculoskeletal issues, from head to toe

  • Member is paired with a physical therapist, who creates a program customized to each member
  • The program adjusts as each member progresses, with check-ins, coaching, advice, and chat available anytime.
  • All exercises can be done at home through a tablet with motion sensors that guide each member flawlessly through every stretch and bend. 
  • Members can access a library of information with insights, advice, and tips

New benefits being added to the program scope for 2024

Technology Support 

Spending time with members in their homes, focused on common household technology projects, including

  • Log in and navigation of the Plan’s portals, apps, custom content 
  • Setup and management of smartphones, tablets, computers, laptops, printers, watches, and mobile apps
  • Organization, setup and troubleshooting of household Wi-Fi
  • Management of digital photos 
  • Operation of “smart” household devices such as TVs, thermostats, security systems, and kitchen appliances

Maid Services

Supporting the Plan’s members with help around the home that includes…

  • Light housekeeping
  • Standard house cleaning
  • Deep house cleaning

Caregiver Support Services

Providing resources to help family caregivers better support members at home with services including

  • Training modules
  • Diverse range of support resources from vetted / credentialed providers
  • VR-Based support and training toolsIn-home respite and caregiver relief services

Financial Management

  • Account Manager receives and reviews bills for errors and consistency
  • All bills paid timely and correctly on member’s behalf from the member’s account
  • Past activity available through portal and paper statements
  • Monthly statement shows budgeted spend vs. actual spend. Also presents dates, amount, bill copies and manner of each payment
  • No computer required; process can be managed via phone, tablet or computer

Advanced Care Planning

  • Comprehensive healthcare planning process led by a skilled facilitator
  • Neutral 3rd party helps facilitate important conversations around medical care options while sharing preferences with loved ones and care teams
  • Structured and supportive workflow for building Advanced Care Plans that support the member’s goals and values
  • Connection to needed social support services
  • Can help capture Wellness and Health Planning requirement data for VBID administration

Cognitive Screening and Coaching

  • Users complete baseline neuropsychological assessment using FDA-registered technology assessment
  • Technology measures user’s responses, response time, and eye movement
  • Users meet with a physician to discuss their scores, cognitive status, and next steps
  • Each patient is matched with a personal health coach to work with for the duration of the program. Through video conferences and asynchronous chat, coaches help participants improve their brain health by working on a range of lifestyle domains
  • Coaches work with users on modifiable risk factors and other cognitive needs
  • Users retest at a specified cadence. Coaches modify plans as needed

Social Support Services

Connecting members with locally available social support services including
  • Support groups, grief counseling & 12 step programs
  • County ride and public transportation programs
  • Faith-based programs
  • Coordination with other covered benefits and social support programs excursions

Virtual Leisure and Travel

 Includes virtual companionship, virtual travel, virtual classes, and events. This service is provided by Loop. 

Loop enriches the lives of seniors globally, connecting them through engaging virtual classes, events, and content, creating unparalleled emotional bonds and easing the pain of social isolation; building a community one human being at a time.
